The medical record

Most of the time, a medical file to be completed will be sent to you by your nephrologist and the healthcare team at your referring dialysis center will provide you with a completed file as well as the laboratory results necessary for your treatment in your holiday center.

The healthcare team will send your medical file by email and / or fax to the center hosting you on vacation.

On the last day of dialysis at your usual center, the nurse will give you a copy of your vacation file as well as a copy of your last dialysis session.
